Sam Nguyen Phan, 57, of Forsyth, passed away on April 14, 2022. Services will be held at Thomas L. Scroggs Funeral Directors on Monday, April 25, 2022 from 9:00 am - 8:00 pm, Tuesday, April 26, 2022 from 9:00 am - 8:00 pm, and Wednesday, April 27, 2022 from 8:00 am - 10:00 am. We will leave the funeral home at 10:00 am to go the crematory.
